After Hours Doctor Services

Urgent Medical care

We provide urgent medical care service that includes and not limited to comprehensive assessment to assess the resident’s immediate health status, conducting a neurological evaluation especially if there was a head injury or if the resident is on anticoagulants and addressing any pain that the patient may need urgent attention to.

Post Fall Reviews

Post-fall reviews are comprehensive assessments conducted after a resident has experienced a fall. These reviews are essential to identify the cause of the fall, assess for any injuries, triage if patient is suitable to be managed in the facility itself or requiring a transfer to the Emergency Department and advice measures to prevent future incidents.

Medication chart management

Our doctors understand that there can be unprecedented changes to the patients regular medications that can not wait for the regular GP’s next visit. We provide service to attend to the necessary urgent updates for medication charts.

Emergency Stabilisation

In times of Emergency, our Doctors provide initial treatment for more serious conditions and assist in arranging transport to a hospital if necessary.

Communication with Regular GPs

Patient’s regular GPs are privided correspondence related to any After Hours consultation in order to keep them informed about any changes to their patients health and management to ensure continuity of care.

Daytime Primary care services

On-site visits and care

Our doctors make regular visits to the facility on a weekly basis to provide in person assessments and treatments. Our GPs monitor the overall physical and mental health of residents and identify any changes or emerging issues early.

Chronic disease management

Our GPs help residents with conditions like diabetes, heart disease, dementia etc. by developing personalized care plans.
Customized Care Plans.

We develop individualized care plans for managing chronic diseases, emphasizing care coordination by liaising with other healthcare providers, such as specialists, physiotherapists, and nurses, to ensure a cohesive, comprehensive approach to resident’s care.

Preventative Medicine

We promote regular screenings and health assessments such as routine checks of Blood Pressure, Diabetes control, vaccinations and immunisations, nutrition assessment and counselling, falls risk assessments and mental health and cognition assessments.

Comprehensive Geriatric Assessments

We arrange regular geriatric assessments to provide in-depth and holistic care for all residents in aged care facilities. These assessments encompass various aspects of physical, mental, and emotional health, ensuring that each individual’s needs are thoroughly evaluated and addressed.

Family Case Conferences

Our doctors are dedicated to improving ongoing patient care by facilitating family case conferences. We are happy to arrange meetings with families and allied health professionals to discuss unmet needs and collaborate for tailored care plans for our patients. These conferences provide an opportunity for open communication, shared decision-making, and holistic support for the individual’s well-being.

We take initiative to educate patients and caregivers on disease management, nutrition, and physical activity to promote self-care and independence.

Prescriptions Management and Medication Reviews

Our GPs regularly review and reconcile all medications to ensure that the medication chart accurately reflects the resident’s current prescriptions.

Our GPs conduct periodic reviews to evaluate the necessity, effectiveness, and safety of each medication.

Costs

Bulk billing will be used to ensure that residents who have a valid Medicare or DVA health card do not have to pay out-of-pocket for clinically required services.

When there are no Medicare rebates available for a specific kind of service, additional private costs can be charged.

VCAT paperwork, insurance forms, letters from lawyers and other third parties, emails, meetings, and calls made when the patient is not present during the patient-doctor appointment are a few instances. These costs are subject to communication and discussion beforehand.
